When you complete a chapter,take a small notebook and write the concepts and formulae together.Always try to explain yourself,where did the formula come from and what's the concept behind it.The more and more your will revise and explain the concepts yourself,you will remember all the concepts and associated formula.Also write down formulae 10 times that you find hard to remember and again explain yourself how did you get it.

Try some following ideas.
1. Write down formulas for each chapter separately.
2. Memorize once all formulas Generally.
3. Try to solve the problems which is related to your formulas for each chapter.
4. Now finally Memorize one more time.
5. Surely, you will never forget. Importantly try to understand concepts while solving problems.

Make a small copy of formulas chapterwise.. very small and thin and read it weekly on every sunday as try to do 2-3 numericals on every formula.. perform this for 2-3 months u will gradully see the results... its slow learning process.
If possible make short snippets card for formulas and read them day wise daily in week for 7 days...
U will remember all of them
